Patient Pathways

What are pathways?

Patient pathways are general guidelines of care for common illnesses and injuries that suggest what events are likely to happen during a hospital stay. Events such as treatments, consultations, medications, diet, assessment, teaching and preparing for discharge from the hospital are mapped on a timeline. Our pathway brochures are easy for patients and families to understand and explain what is likely to happen while in the hospital. Pathways represent the most advanced care and are included in the patient's medical record.

Patient Pathways are developed for each specialty procedure, to ensure consistent standards of clinical care are maintained for all patients at any hospital they may be treated at within the Choice system.

Regional clinical pathways have emerged as a strategy to facilitate the management and delivery of consistent, quality clinical care across the Network in a time of constrained resources. The Grey Bruce Regional Clinical Pathways Project began in early 2002 and built upon best practices found in literature and in institutions across the country to enhance the coordination of care across rural hospitals and community health service a
